JFCS contracts with Ramsey and Hennepin Counties to provide case management to individuals with disabilities or chronic illnesses who need a certain level of care that have qualified for the Minnesota’s home and community-based waiver programs. Clients are assigned to JFCS directly from the county in which they have the waiver and clients who are already on a waiver can also select JFCS as their case management provider.
JFCS is committed to manageable caseloads so that waiver case managers can give their clients the attention they require and deserve. Client-centered care is always the primary focus. The diversity of JFCS waiver case managers also enables care that honors each client’s unique background. Several of our staff are bilingual or multilingual. If there is a preferred language that our staff are not fluent in, we provide interpreting services over the phone and at in-person meetings at no cost to the client.
