The Alzheimer’s Association leads the way to end Alzheimer’s and all other dementia — by accelerating global research, driving risk reduction and early detection, and maximizing quality care and support. We work on a national and local level to provide care and support for all those affected by Alzheimer’s and other dementias. Resources available nationwide are our 24/7 Helpline (1.800.272.3900), care consultation, support groups, educational programs and dementia care resources.


Best Self Therapy carries a mission of instilling hope in the way grieving adults view, talk about, and navigate grief; allowing them to live a better life as their best selves.


Brighter Days Family Grief Center is a statewide nonprofit Grief Center. We provide free psychosocial, logistical, and financial support for all ages of grievers. This includes free grief counseling, support groups, youth and family grief camps, financial assistance, legal guidance, and logistical support. We also provide community support to schools, hospices, and local organizations including trainings and after-crisis care.


Conversations with Kelly is on online and in person platform to provide discussions and support focusing on grief and chronic/terminal illnesses


At English Rose, our mission is to optimize wellbeing. What makes this a challenge is that the definition of wellbeing is different for every person. Moreover, the factors that contribute to an individual’s wellbeing change over time as they age or their disease progresses. What makes English Rose so unique is our ability to stay in tune with each individual and adjust our approach to care accordingly.


Giving Voice Initiative – Choral Communities of Joy – We operate five choruses in the Minneapolis area. Inspiring Choruses around the world – through training, the Giving Voice Toolkit, and a collaborative network, GVI is leading a worldwide movement.
Changing the Narrative – Giving Voice educates and advocates for living well with dementia and the benefits of singing in community. Our program at Giving Voice understands the importance of support for care partners. We offer a community where care partners can find solace, share experiences, and gain valuable insights from others who understand their journey.


Guardian and Conservator Services is a limited liability company in the state of Minnesota serving all 87 counties. We provide a range of fiduciary services. Our professional care/case Manager Services can assist as a Guardian, Conservator, Personal Representative, Social Security Representative Payee, VA Fiduciary Services, Estate Maintenance and Liquidation, Trustee, and Power of Attorney and other fiduciary services.

Lorenzo’s House is a non-profit organization offering free VIRTUAL programming with a focus on the sons, daughters and their families of younger-onset dementia (symptoms before 65 yo). Together we are shifting the narrative: curing isolation, building community & driving dementia justice through our 3 pillar programs.


The Mayo Clinic Alzheimer’s Disease Research Center promotes and conducts research and education about healthy brain aging, mild cognitive impairment, Alzheimer’s disease, Lewy body dementia, frontotemporal dementia and other dementia conditions. Ultimately, center researchers aim to prevent, delay and possibly cure Alzheimer’s disease and other dementias. In addition to doing research, our staff provides care and support for people with Alzheimer’s disease or dementia, their families, and their caregivers.


Minnesota JCC – Inspired by Jewish values and culture, we ignite the human spirit through programs and services that promote well-being, foster inclusivity and strengthen the entire community.


Myrna Marofsky – A resource for care-partners to adopt a mindset of Care-Living.


Navigating Elderhood – We provide a highly personalized, white-glove care advocacy service for individuals and their families. Our team of medically trained professionals acts as dedicated advocates, ensuring seamless, person-centered care for clients who wish to remain in the comfort of their own homes throughout the aging process, even amidst complex medical needs or declining health. Our focus is on empowering individuals to live with dignity and independence at home, while providing peace of mind for their loved ones.


Open Circle Adult Day Services – Our mission is to foster fullness of life for older adults. We are an adult day program designed to provide support to individuals with changing physical, cognitive and or social abilities and help them achieve their goals by attending to mind, body and spirit in a supervised and nurturing setting. Our centers offer therapeutic programming, a daily exercise program, leisure activities, social connections, and nursing oversight.

The Senior LinkAge Line is a free, statewide service of the Minnesota Board on Aging in partnership with Minnesota’s area agencies on aging. The Senior LinkAge Line helps older Minnesotans and caregivers find answers and connect to the services and support they need.


Sholom provides compassionate support to adults across the continuum of care: Independent Living, Assisted Living, Memory Care, Skilled Nursing, Home Care and Hospice. We are dedicated to helping individuals live life to the fullest in a welcoming and inclusive environment.


So Many Goodbyes – We inform, inspire and affirm everyone affected by Alzheimer’s or other dementia through mentorships, workshops and other outreach, including the “Hello Alzheimer’s” podcast. We believe that everyone CAN find joy — even during this difficult journey — if they’re able to access the resources that DO exist in our greater community. We communicate how dementia represents a national, even a global, crisis — such that we DO need to support each other. Bottom line and most importantly, we remind everyone on this journey or touched by any form of dementia — “You are not alone.”


Still Waters Mediation – We specialize in civil mediation, with a particular focus on disputes between adult family members that may arise when caring for aging parents, the appointment of guardians and conservators, planning estates, and during the probate process, including disagreements over the distribution of assets. We understand that navigating the aging process, supporting a loved one through end-of-life decisions, and dealing with loss can be incredibly emotional and challenging. Our professional and empathic mediators are here to provide compassionate support during these difficult times.


SYNERGY HomeCare – We are a non-medical home care provider. We have a team of approximately 90 caregivers who work in our clients’ homes. We can provide respite for the family caregivers. We have no minimum hours and no contracts.
