JFCS Care Coordination allows Sam to share good times – not paperwork – with her mother

I recently received an intake call from “Sam,” a caregiver who had many questions and areas where she needed assistance. Like most caregivers, Sam was focused on her loved one and her needs. Her mother, “Abby,” faced medical problems and needed help navigating the medical system. Coordinating insurance coverage, pharmacy updates, and medical appointments became too much for Sam, who already did so much for her mother.

 

We made a plan for Abby to begin JFCS’ Care Coordination service. Care coordinators are matched with clients and become the primary coordinator for the delivery of services, individualizing and adjusting services when needed and connecting to family members, friends, clergy, outside agencies, community professionals and other programs at JFCS.

 

Now I accompany Abby to medical appointments, where I advocate for what she needs, help coordinate her care, and keep her daughter in the loop. I assist Abby in considering her treatment options and make sure she has all the information she needs.

 

Now Abby and Sam can spend more time together enjoying fun activities. What were once weekly visits spent on paperwork and driving to doctor’s appointments have now become enjoyable visits and outings. Sam can be rest assured she has a support network available in her role as a caregiver, so she can still enjoy her special bond with her mother.
